Rx Partners-LTC in collaboration with Community LIFE for the development and implementation of a Medication Cost Reduction Program.

The RISE Program (Raising Immunization Safety and Effectively) was awarded the 2008 Immunization Excellence Award under the Healthcare Worker Campaign category from the National Influenza Vaccine Summit (NIVS). The NIVS is a joint collaboration between the CDC (Center for Disease Control) and the AMA (American Medical Association).

Rx Partners-LTC was an active participant on the UPMC Seneca Place Care Transitions Committee which developed and implemented a transition of care program for residents upon discharge from the nursing facility to the community or hospital. Rx Partners-LTC created a protocol to assist nursing with medication discharge planning which included an education program for residents/families. UPMC Seneca Place was awarded a grant from Highmark for improvements in clinical outcomes of residents discharged to the community or hospital.
